This checklist template for article retraction has been developed to assist editors in preparing a retraction notice and to help researchers analyze retractions.

The form consists of check-box fields and space for clarification in order to indicate who is retracting the article, the reason for the retraction, and the history of errors/statements/concerns. A free-text field allows the editor to add any information deemed useful, and the final field for the retraction notice is provided for summarizing the details of the form in the text that will be published as the formal retraction announcement. (download)
